Appointment Checklist

☐ Click “Request Appointment” button (above)

☐ Select the counselor and date for your heart counseling session

☐ Submit intake paperwork through the online portal, once your appointment is confirmed

What happens next?

Intake paperwork will be emailed to you within 3 business days of your submission and we will contact you to answer any questions you may have.

Any cancelations or “no-shows” (within 24 hours of the appointment) will be charged the full cost if it is not an emergency.
